your child was given a confirmation information sheet to take home today. You need to complete it and return tomorrow. In the unlikely event that your child did not put the sheet in his/ her bag as requested I am attaching a copy so that you can supply the information required for tomorrow. You do not need to print the page off simply write the information as it is requested in the form onto a piece of paper. Mothers are reminded to give both their Christian name and maiden name if applicable.
